
Ashwagandha root extract appears to nudge thyroid hormone production upward. In one small 8-week trial in people with subclinical hypothyroidism, 600 mg per day of KSM-66 lowered TSH and raised T3 and T4 compared with placebo. That makes it a plausible adjunct for a sluggish thyroid under medical supervision, but it is not a replacement for levothyroxine, and it is a poor fit for hyperthyroidism or Graves' disease, where extra hormone is exactly what you do not want. With Hashimoto's the picture is mixed: the thyroid effect may help, but a theoretical immune-stimulating effect argues for caution. Ashwagandha shows up in almost every stress supplement stack, and for good reason: the trial data on cortisol and perceived stress is better than for most herbs. But if you have a thyroid condition, there is a second effect you need to know about before you add it. Ashwagandha does not only calm the stress axis. In several studies it also pushed thyroid hormone levels upward.
For someone with an underactive thyroid, that sounds like a bonus. For someone with an overactive thyroid, it is a problem. And for the millions of people taking levothyroxine every morning, it raises a practical question: does adding ashwagandha change what the medication is doing?
Here is what the research actually shows, condition by condition, and how to use ashwagandha safely if your thyroid is part of the equation.
How Ashwagandha Interacts With the Thyroid
The thyroid works on a feedback loop. The pituitary gland releases thyroid-stimulating hormone (TSH), the thyroid responds by producing thyroxine (T4), and tissues convert T4 into the more active triiodothyronine (T3). When hormone levels are low, TSH rises to demand more. When they are high, TSH falls.
Ashwagandha appears to act on this loop in two ways, according to the National Center for Complementary and Integrative Health and the animal and human studies behind its summary:
- Increased hormone output. Rodent studies from the late 1990s found that ashwagandha root extract raised circulating T4 and, at some doses, T3. The proposed mechanism involves withanolides stimulating thyroid tissue directly and reducing oxidative stress inside the gland.
- Lower cortisol. Chronic stress and high cortisol suppress the conversion of T4 into T3. By blunting cortisol, ashwagandha may indirectly improve how much active hormone your tissues see.
Neither effect is large in absolute terms. But the thyroid is a system where small shifts move symptoms, which is why the direction of the effect matters so much for each condition below.
What the Human Research Shows
The most cited human study is a 2018 randomized, double-blind, placebo-controlled trial published in the Journal of Alternative and Complementary Medicine. Fifty adults with subclinical hypothyroidism (elevated TSH, but normal T4) took either 600 mg per day of KSM-66 ashwagandha root extract or a placebo for 8 weeks.
Compared with placebo, the ashwagandha group saw:
- TSH fall by roughly 17 percent
- T3 rise by roughly 41 percent
- T4 rise by roughly 20 percent
Those numbers look dramatic, but keep them in context. The trial was small, lasted only 8 weeks, was funded in part by the extract's manufacturer, and enrolled people with subclinical disease, not people already on medication. It has not been replicated at scale. What it establishes is a plausible direction of effect, not a treatment.
A second, less direct piece of evidence comes from a 2014 study in people with bipolar disorder who took 500 mg per day of Sensoril ashwagandha for 8 weeks. The trial was not about the thyroid at all, but the researchers noticed that T4 levels rose in several participants. That is a useful reminder: the thyroid effect shows up even when nobody is looking for it.
Finally, there are individual case reports of thyrotoxicosis (excess thyroid hormone) in people taking ashwagandha, including at least one in a woman with no prior thyroid disease. Case reports prove nothing about frequency, but they do show the effect can be clinically meaningful in some people.
Hypothyroidism: A Possible Adjunct, Not a Replacement
If you have diagnosed hypothyroidism and take levothyroxine (sold as Synthroid, Levoxyl, Euthyrox, or Eutirox depending on the country), ashwagandha is not going to replace your prescription. Levothyroxine provides the hormone your thyroid cannot make. Ashwagandha, at best, encourages a gland that still has some function to produce a little more.
Where it may fit is at the margins:
- Subclinical hypothyroidism that your doctor is monitoring rather than treating. This is the population the 2018 trial studied, and it is the most defensible use case.
- Stress-related symptoms that overlap with hypothyroid symptoms: fatigue, poor sleep, low mood. Ashwagandha's cortisol effect may help here regardless of what it does to the thyroid.
What it should not be is a reason to skip or lower a prescribed dose on your own. Thyroid medication is dosed against lab values, and changing the inputs without retesting is how people end up either under-treated or over-treated. Hyperthyroidism and Graves' Disease: Skip It
This is the clearest recommendation in the article. If your thyroid already produces too much hormone, whether from Graves' disease, a toxic nodule, or thyroiditis in its overactive phase, ashwagandha is working against your treatment. Every mechanism described above pushes hormone levels in the wrong direction.
Symptoms of excess thyroid hormone include a racing heart, heat intolerance, tremor, anxiety, insomnia, and unintended weight loss. Several of those are the exact symptoms people take ashwagandha to fix, which creates a trap: someone with undiagnosed hyperthyroidism reaches for a "calming" herb and feels worse, then assumes they need more.

Hashimoto's: The Autoimmune Question
Hashimoto's thyroiditis is the most common cause of hypothyroidism in countries with adequate iodine. It is an autoimmune condition: the immune system attacks thyroid tissue, and the gland slowly loses function.
This creates a genuine dilemma. On one hand, the hormone-raising effect could help a Hashimoto's thyroid that still has partial function. On the other, ashwagandha is often described as mildly immunostimulating, and the conservative view is that stimulating an immune system already attacking your thyroid is unwise.
The honest state of the evidence is that this concern is theoretical. There are no trials showing ashwagandha worsens Hashimoto's antibodies or accelerates gland damage, and some clinicians use it in Hashimoto's patients without apparent problems. But there are also no trials showing it is safe in this group specifically. The 2018 trial did not report antibody status.
If you have Hashimoto's and want to try it, the reasonable approach is medical supervision, a lower starting dose, and baseline plus follow-up labs that include thyroid peroxidase (TPO) antibodies, not just TSH.
Ashwagandha and Levothyroxine: Timing and Monitoring

Can you take ashwagandha with levothyroxine? Many people do, and there is no documented direct chemical interaction the way there is with calcium or iron, which physically bind the medication in the gut. The concern is pharmacodynamic: two things pushing thyroid hormone in the same direction.
Practical rules that keep this manageable:
- Separate the doses by several hours. Levothyroxine is absorbed best on an empty stomach, 30 to 60 minutes before breakfast, away from other supplements. Ashwagandha is better tolerated with food. - Recheck TSH 6 to 8 weeks after starting. That is the standard interval for any change to a thyroid regimen, because it takes that long for TSH to settle at a new equilibrium. If TSH drops below range or T4 rises above it, your doctor may reduce the levothyroxine dose or ask you to stop the herb.
- Do not stack it with other thyroid-active supplements without a reason. Iodine, selenium in high doses, and "thyroid support" blends containing bovine glandulars all act on the same system. - Tell every prescriber. Ashwagandha also mildly amplifies sedatives and has rare liver injury case reports, so it belongs on your medication list.
Signs You Are Getting Too Much Thyroid Hormone
Whether you take levothyroxine, ashwagandha, or both, the symptoms of excess thyroid hormone are the same, and they are worth memorizing because they arrive gradually:
- Resting heart rate noticeably higher than your normal
- Feeling hot when others are comfortable, or sweating more
- New or worsening anxiety, restlessness, or trouble sleeping
- Fine tremor in the hands
- Unexplained weight loss or increased appetite
- Loose or more frequent bowel movements
If two or more of these appear within the first couple of months of adding ashwagandha, stop it and get labs done. The effect is reversible: in the trials, hormone levels drifted back toward baseline after the extract was discontinued.
How to Take It If Your Doctor Agrees
For someone with hypothyroidism or subclinical hypothyroidism who has cleared it with their prescriber, a conservative protocol looks like this:
- Baseline labs first. TSH, free T4, and ideally free T3 and TPO antibodies, so you have a reference point.
- Start at 300 mg per day of a standardized root extract (KSM-66 at 5 percent withanolides is the most studied). Take it with dinner, well apart from levothyroxine.
- Hold for 4 weeks, then go to 600 mg only if you tolerate it and want the dose used in the trial.
- Retest at 6 to 8 weeks. Bring the results to whoever manages your thyroid.

Log the dose, timing, and how you feel each day. Thyroid symptoms shift slowly, and the difference between "the herb is helping" and "my levothyroxine dose is now too high" is easy to miss unless you have a record to compare against your labs.
